Road Name Reservation Form

Per the Gallatin County Addressing and Road Naming Policies and Procedures Manual road naming (2.04.04) has guidelines that should be carefully reviewed before beginning the process of choosing a road name. The purpose of reserving a road name is for subdivisions or personal properties where it is known a road name will eventually be assigned. This is not a requirement for having a road name approved that process is under road naming tab. This helps guarantee that the road names chosen will stay with that area, if you do not reserve the road there is chance someone could use the road name. If you are unsure of the road name suffix we can assess what the correct type would be based upon layout of the roadway. Once the road name is approved by us we will file it with reserved road names and will honor it for 1 year with possibility of an extension. 

Please list all roadnames you would like to use for the specified area.
Please provide 2 or more alternate road names, this is a whole new road name not just a change of suffix. If we have already talked through the road via email or phone then please write; Discussed over phone/email.
Please provide a layout of the road/s in reference to the property. Please include surrounding road connections if that applies to any of the roads. If there are multiple roads please provide a key for understanding on which roads are in place already and which are tying to be reserved. If we have discussed the roads there is no need to attach anything.
